The simfiles set the background to ".mp4", whereas the video file is ".avi". I don't know if there are versions of StepMania which compensate for that, but Outfox 0.4.16 does not.
Also, I believe measures 12, 14, 19, 36, 38, 43, 58, and 75 should be triplets (green notes) instead of 16ths
