Is it possible to get a black background before the song starts? If so, what should I put in the .sm file?

All you have to do is make a full black drawing in any paint program and label it (insert song title)-bg.png. You don't even need the song title, just a legal name for the file. Put it in the song folder and change the .sm file like so:
#BGCHANGES:0.000=sample-bg.png=1.000=0=0=1;
0.000 is the beat in which the bg change starts. I believe you can have a negative beat, such as -1.000, you can try that.

Doesn't seem to work. Any other ideas?

Are you using SM 3.9 or SM 5?

If you're starting the video at the beginning of the song, just have the black background in the folder with the correct dimension ratio (4:3 or 16:9, depending on the theme) and make sure there isn't another one in there. Stepmania should load the black background automatically before the song starts.
