A bit late here but...
Round 1 Puente Hills's Facebook and the official Hatsune Miku Facebook page confirm Project DIVA Arcade is present for a one month location test (running until March 17/18). It is four credits ($1, discounts for mass purchases available) to play. The location test began on February 14.
Initially online passes were be provided free for those who took a survey but they ran through all 200 cards in three days. The machine can still be played online via Banapass or privately acquired Diva.NET cards.

Also, this report has some photos and a video of the test in action. It also, oddly, reports that if the game gains a permanent spot at Round 1, it wound be available beginning April 21.

Per the Round 1 Facebook page, Project DIVA Arcade has migrated to the Moreno Valley location. Cards are once again being handed out, or you can presumably use a Bana online pass.
