Same here. I have the same exact problem with both the DD theme and the HS theme. I was able to fix this problem with the HS theme by switching back to version 3.9 (down from Plus Redux). Although it fixed the problem, one other visual issue happens on the main menu.
If someone knows a fix that allows you to keep it installed to a Plus Redux version of Stepmania please share. It makes using the IIDX themes more of a hassle to switch to.

2nd line of great (yellow colored) -- PERFECT
3rd line of great (no color) -- GREAT

Also, remember that in IIDX the highest judgment grade is the "shining" Great (if you enable the judgment counter it appears as "PGREAT")
