I saw this machine at Tilted 10, Willow Grove Park Mall, PA. I've visited a couple times earlier this year and didn't see it then.
From what I've found online, it's some kind of bootleg of DDR Extreme? The machine was not functioning so I couldn't tell what it actually was playing.

AWESOME ACTUAL CLEAR FOOTAGE OF THIS DDR EXTREME BOOTLEG!!! 👏 That bootleg is now hard to find in the U.S. because of multiple Tilt arcade closures (Tilt's parent Nickels & Dimes was one of the suppliers for this extreme bootleg) and small lesser known arcades replacing them with proper exported Extreme, or its successors, or removing it as a whole.

Yes, it's a bootleg of Extreme and to my knowledge there were a couple different versions of it. The earlier versions had the freeze arrows show up as blocks , while later ones are basically functionally identical to vanilla DDR Extreme besides all of the silly looking aesthetic changes.
