Bumping this thread to provide one of the requested banners.
It turns out that this banner was also present in the data for the first Ultramix game. As for the other requested banners, I don't have them.

Thanks so much for the banner. Hopefully, someone will have the other 4 banners and upload them.
And yeah... most graphics from the Wayback Machine archive are broken since they weren't saved for some reason...
