Which was taken from extreme AC, on locked machines TMS works this way, and that was released before 7th mix CS.
Yeah I recall playing on a locked Extreme machine and Trip Machine Survivor shows up for Final Stage with good enough grades. AAing this showed Paranoia Survivor and TLoM as Extra Stages, otherwise TLoM shows up for ES exclusively,

Also, Roulette put LOM in a random place on the songwheel. I thought Trip Survivor appeared on second stage?

Honestly, I'm not 100% sure of the requirements but I do remember unlocking the boss songs during a game.

I can attest to that. My only recently unlocked machine had the boss songs appear (minus PSM and TLOM) on the 3rd stage after getting decent grades, including standard paranoia and the trip machines that i've never bothered playing.

The Captain Jack song was once a 9 too. These pics come from the sideart for Max 2 arcade. Found some full size scans of these a while back. And Maximum Overdrive didn't have it's remix subtitle on the song wheel as well during that test location Aaron posted pics of too. Yet some other songs did have theirs. Also I think some of the beta courses had 0.5x or something on some of the songs too. I think Dead End was one? I think Stay - Tess had a bit different song banner too?
DDR Extreme having Marvelous in normal gameplay, Sakura as the encore extra stage instead of DDR. I think there was even what became Fascination Maxx in supernova on this too at test? But I'm pretty sure that was just a rumor. How the Dam Dairim oni steps where for Give It Up by Captain Jack too in Extreme arcade. Just trying to remember some stuff here. I'm sure some of this is just a rumor. But just posting it all just in case. Also I don't know of a song in DDR Max 2 arcade that has this heavy groove radar. I think I heard one time someone said it belonged to Flash In The Night?

At the 55% version of the Extreme Location test, PARANOiA survivor and The Legend of MAX were the extra stage songs. MAX to the XXXimum was just a joke amongst japanese players who used to make up fake songs when the location tests were about to start for the games.
When this would happen it was often pretty irritating figuring out which songs were legit or made up. Tyma can attest to this, he, Malcolm (from bemanistyle) and I used to get together online when LTs started and try to get all of the songlists together for the new games.
I still have a whole bunch of images from the DDR EXTREME location test and Give Me Up wasn't present on the machine either. Both that and "Take Me Out To The Ball Game" from Dancemania SPORTS were rumors to be included before the test started.

Wasn't Paranoia Survivor supposed to be on DDR Max 2 at first? I remember hearing something about that too a -long- time ago. And on the official playstation demo for DDR MAX USA (the red one with the Sly Cooper logo I think) had samplestart data for Aint It Good from beatmania,Kakumei, and I think Nemesis - DJ Setup too. Note that this data was only on this version of the DDR MAX USA demo too I think. Also the song banner for Take It To The Morning Light was found in the ultramix series too. Along with a lot of other song banners found through the old ultramix sites I think. Crazy In Love - Beyonce and at least a few others.
Edit: Wasn't that version of Kind Lady rumored to be on DDR MAX 2 USA as well? I think it could have been like the DDR Extreme Memories code thing too.

Yes, the five banners found were for:
g.m.d. / DJ Mazinger feat. Muhammad
Crazy In Love / Beyonce
Scandalous / Mis-Teeq
Get This Party Started / Pink
In My Dreams / Noemi
As we know, Crazy In Love was put in, but as a cover by wavegroup. g.m.d. is from the CLUB versions, but the other songs were never put in any game.

I'd happily play Kind Lady Interlude at 178bpm if there were a load of 16th runs thrown in to go with the lead synth. On a Difficult "5-foot" chart of course
