Monkey Business still only unlocks on Extra Stage, but it's the full set of charts no matter what your difficulty average is.
As for the Doubles charts, Another Phase is 7 on
(265 steps), and 11 on 
(333 steps), didn't try to go for

Both charts are a lot of fun, and the song title is very apt.

I registered my eAmuse card and got this. This page lists every single DDR session I play and then what percentage I've progressed for each song, and the total progress. It seems that once I hit 100% for a given song, it'll become playable. Though I don't know if the counter starts over or if it's permanently available after that.
I haven't touched any other Bemani titles with this card.

A song is permanently unlocked once you reach 100% (and it unlocks for all 6 games doing the Bemani Gakuen)
Also, each pair of artist is linked to a certain game (by playing this game the clear rate will increase faster). In DDR's case, it's of course Tag/UI song (5% per play without any bonus, can climb up to 20% at once as far as I saw with the bonus)
