Do I have to play/clear it on Heavy? I played it once on Standard and didn't get version B. I don't really want to have to clear it on Heavy, because that's something I probably can't do without a controller right now.

If you didn't get it after playing it on standard then that means I really lucked out! 
If my method was a fluke then the method to unlock them in training and edit mode would be to select and clear it in Standard Mode until you play the version you're looking for.
Edit: Aegis, stop flamebaiting. It's unnecessary and it's getting highly annoying.
