I had just gotten good enough to beat Drop Out 
on EX Home, and I went to the arcade for probably only the third time in my life. My brother was up, and the other player selected this song. I wasn't sure he could pass it, so I asked him if he'd let me take over, and he did...failed it before I even got a fourth of the way through it.
Oh, and more than a year later, I was encouraged by the people watching me to do MAX 300, since they noticed my skill at 9-footers. I didn't even make it through the first 8th swarm, and all those people were watching me...
Then, there were the times at home when I sprained my ankle playing songs like In The Navy and Xenon with my family watching...

My most epic fail was...
Failed when trying to play Arrabiata double no bar.

It's not a story of Epic fail but I have also slipped off the pad. I was playing Max 300 SMMM Doubles Basic but it had been my third time trying and I somehow slipped on my right foot and stumbled off the pad. Luckily the mall was almost empty and no one saw me in the food court. I thought it was pretty embarrassing since I had NEVER fallen off the pad before.

It wasn't for me, but when I was in New York over the weekend, my friends challenged me to a one-on-four DDR match. I would play one pad, while my four friends took one arrow a piece. I was allowed to choose the song.

I wiped all four of them out, and won the bet they had made with me. They didn't live it down the rest of the trip.

I think we have to talk about OUR OWN failures.

I got cocky one day in front of friends and picked this chart on a dare. I barely managed to avoid walking off the pads, and oh God was I flailing all over the place trying to keep my balance. It was all in good fun.

I was trying to AAA Trip Machine Climax all day at the arcade 2 weeks ago. I was getting black flagged all day on the song (at LEAST 5 times). I played it again and I really thought I was going to get the AAA but then on one of the last notes I got a great. I started to flipping out about it, played it again and on the first note I got a great so I just grabbed all my stuff and just left. I made a big scene on my way out and people were staring at me. I dont think I have made such a big deal out of a video game, but it kind of bugs me now knowing that I will be that guy no one forgets about that flipped out over a round of DDR.
Also one time I was freestyling R3 in front of a crowd and I failed the song because I started one of the moves on the wrong foot and arrow. Got a few laughs (I was laughing too).

Im a reletavely experienced doubles player, and was playing Jane Jana. I was getting really into it and started really flowing to the music on the crossovers across the pad.
On one of my twists, I tripped over my own foot and hit the floor. I tried to get up quickly and play again, but the song failed.

I decided to play double, so it's like 4 euros the game.
Yeah this day i wanna play double so bad. I can clear 5/6 feet.
I used to start by dynamite rave

But the life bar here (i knew it) was a fucking bitch.
I combo'd the beginning of the song, fucked up one part and got like 4/5/6 miss in a row (i dont remember) so i failed.
I could finish the song (failed end of song) combo'd the rest of the song...
I was so pissed off. The life bar was ridiculously set to 6 or 7 i think.
+ the high price of the game...

I made a little collection of picture when i do the same song in a row ahah.

Oh and back in November last year I got one great on the last arrow before the jump on MAX 300. I was so pissed off I didn't play for like 2 weeks. I did end up AAAing it recently and got it on film, but that was all done at my house not the arcade.
That is the only time I think I ever made a scene at a arcade being pissed off, and I have had a lot of BS happen to me.
There was also this one time I broke the down arrow on the machine playing normally (it got jammed) and I walked off the pad and everyone that was watching me was like wtf? I turned around and just said "sorry I just broke the machine" and then I left to play something else (Sangokushi Taisen). That was at Gameworks when the DDR machines were still on the 1st floor so it was well over a year ago. It wasn't really embarrassing but it was pretty funny in my opinion.
