Extra 4 keys? I think you're forgetting the people that hook an actual pad up to stepmania.
As for doubles only:

hmmm. I never knew people would actually only put doubles (or couples but it's pretty much the same idea). Also I do know people do hook up their pad to stepmania but I'm just talking about people who play the keyboard

Same here, but I use a slightly different setup. It's WASDTFGH so I can use two fingers.

It's really easy to play Doubles on keyboard if you know how to program the keys (which anyone who regularly uses a computer should be able to do).

Of course, there's an additional learning curve when using extra keys, but once you're accustomed, it's the same button mashing as Singles.
