I'm not the best at DDR but I find myself sometimes feeling the same thing when I'm trying to practise 9's and 10's on the EXTREME cab, and 15+ songs on DDR X. The main thing I would say is - you can use the bar. It's not going to make you worse of a player by using fair help to get better. To crossover, just turn 90 degress right of left and hold with one hand. I would also agree with Tyma in a different way. The gist of this is 'trial by fire.' I got good at the game by playing the hardest songs on EXTREME so much that when I wasn't paying attention, I was getting much, MUCH better at being accurate. That seems to be the hardest thing.
Play the hardest song you can without the bar. If and when you pass it, try it with the bar and you'll see how much you've improved. Worked for me. Final thing, if you're at an arcade version and there's a song that you want to clear that is a 17, 18 or 19, I would suggest playing it first. I don't know about your arcade, but the harshness of the life bar goes up with each clear at mine. Keep playing, and you'll be good.

A little off-topic: The original poster could pass standard first try as in was able to nail eighth note patterns? That is insane! I couldn't pass two-footers when I first tried DDR. Can most people pass four-footers first try or something?
More on-topic: I can't really give any solid advice since I am nowhere close to the level of the original poster. I guess I can mention that it might help to take breaks from playing when no progress seems to be made -- maybe playing too much is as ineffective as playing too little, possibly? Also, I have heard that not working on bar-style play consistently can screw up one's technique. I don't know if that applies to this thread, but I thought I would mention it. Playing on keyboard and/or PS2 controller might also help, especially with getting used to different speeds and BPM's. But, don't take my advice as gospel as I am not the most experienced player ever.

Actually, I can PFC (or get close to it) almost every DDR song using stepmania keyboard. My reading is fine, I am just unable to pass harder 18-19 footers
VD Heavy? I can pass pretty consistently, and I am getting closer to a B (went from 700000 to 760000 over time)
But Eternal Love Mix Cha? Nope. Can't pass it because of cross over step jumps.
I can almost pass Possession Cha, it is simply a matter of stamina there. (I have no energy by the time I reach the 12th note cross over jumps at the end. I can make it through that ridiculous 8th note cross over section at least. :/)
I have (since starting this thread) gotten a bit better at step jumps on slow songs (Be Lovin most notably), but I still can't quite do those high BPM songs.
However, my biggest disappointment is that I can PFC 7-12 footers (or very near it), but 13-14 is about the limit of my AA ability, and at 15s, I can no longer AA anything. I haven't even FCed anything beyond that point. Between 16-18, I have never gotten an A. And I have never passed a 19.
My point of discussion was mostly a suggestion that I will probably never get much further than I am. Most of the advice suggested, I have done already. (I have no barred and passed Max 300, SMM,, Legend, Sakura, Pierce the Sky, Paranoia Survivor, Paranoia Respect, ROppongi C, etc)
But my scores dropped from 880000~ to a range of 680000-820000, depending on the song. I just can't seem to break through. :/
Yes. My first song was Butterfly on Light, and when I passed, I played it again on standard. I then Played it on standard about 10 times in a row (those poor people who had to listen to that earworm for half an hour!)
After that, I tried it on Heavy, failed, and then tried other songs on standard. I found light really, really boring. Eventually, I moved on to Heavy songs with my friend, Holic, suggesting that I should try to pass Xenon on standard first. (That took a couple of tries! It is a very tricky song and a great introduction to what Heavy songs are capable of!)
I got my first Extra stage from Kind Lady on Heavy within my first few months of playing. I got my first AAA from Freckles Heavy within my first year of playing (when exactly? I have no idea.) Side note: I got under 5 great on R&P KOG before I passed Max 300 for the first time.
