OITG and stepmania automatically overwrite some of the data depending on where you put it, I believe. You'll have to hunt around the boxorroxxors.com forums for it, it's under the general open itg forum somewhere.
Still, if you can get sm 3.95 then find the itg 2 official arcade skin and install it, you should be able to use the course fine. I mean, it still appears as 5 or 6 songs or whatever but it works well enough.

I'm pretty sure you can throw the contents of keyboard.ini into static.ini.

Those are problems with the theme; complain at the person who made it. OpenITG's default theme ("home") has an Exit option on the main page and a Config Key/Joy Mappings in the operator's menu.

OpenITG is an open-source program, what do you mean by 'locked down'? That you're using a theme whose metrics were copied from the R2 arcade theme? Because that's still a theme issue and still not a program issue.

Keymaps.ini is only read and written by the input mapping system, so no. I've never gotten any manual entries to work, either, so I'm not sure if it's possible to add stuff into it manually.

Necrophantasia is too hard if you havent got solo singles option enabled, otherwise the arrows dissapear =3=
