Check your current theme's metric.ini if there is NextTheme code. If yes, you should delete it so that the one from _fallback should work
I did it on a clean install of themes folder with just the default ones sm5 comes with and it still doesnt work for me.
MenuLeft and Menuright are mapped aswell.

Looks like they are not working anymore (it says deprecated). The other way to trigger them though is in below:
Add these 3 lines to the bottom of your _fallback theme's metric.ini:
[ScreenTitleMenu]
CodeNames="ChangeTheme"
CodeChangeTheme="MenuLeft,MenuLeft,MenuLeft,MenuRight,MenuRight,MenuRight,MenuLeft,MenuRight"
Then download and put this file to your _fallback theme's BGAnimations folder:
https://openload.co/f/LW3Nb0KZ9ME/ScreenTitleMenu_overlay.lua
During title screen, press MenuLeft x3, MenuRight x3, MenuLeft, MenuRight to switch to next theme.
Add these 3 lines to the bottom of your _fallback theme's metric.ini:
[ScreenTitleMenu]
CodeNames="ChangeTheme"
CodeChangeTheme="MenuLeft,MenuLeft,MenuLeft,MenuRight,MenuRight,MenuRight,MenuLeft,MenuRight"

Hello, I'm glad you enjoyed my theme, I always try to make them as faithful to the original. the operator menu is based on the original games, if you wanna access SM options, just go into the secret option.
