Is it even available for extra stage on the normal difficulty? What am I doing wrong? After I play the third song, it just goes to a final evaluation screen combining my notescores from all three songs, then credits, game over, menu screen, no extra stage.

To get any special extra stage, you also need to fufill the normal extra stage conditions on your last song as well (except for CS Empress):
Level 4-: Perfect combo (no goods) [I haven't ever seen this done, and it may only be the case for CS Troopers and later]
Level 5,6: Full Combo
Level 7: End with 100% lifebar (I've heard once that just hard clearing may be enough, but that isn't confirmed)
Level 8+: Simply clear the song (It can be an Easy clear for a normal extra stage)

Glad to see I was remembered, lol. You guys are awesome.

I've been preoccupied with college and whatnot lately; I was just starting at a junior college when I was the most active here, and didn't have much time for anything when the classes started to get harder. I've also lost a little interest in most rhythm games like DDR since the local arcade's SuperNOVA is probably all I'll ever experience until I move out of Albany GA someday, but still only a little; I am still a die-hard ITG fan, though.
Back on topic, it was what GameFAQs and then Wikipedia said to do that I already tried twice; I'm going to try doing what Meseki said, but no promises that I'll get back with anyone soon about this, because I have no hope of perfect comboing anything at any difficulty as of now, and I rarely even full combo 4's, as opposed to full-comboing a 5.
