It's a heavily modified StepMania with the DJ TROOPERS theme.

I think that this is a homemade theme and somehow they manage to put it into DDR Extreme Arc cabinet.

The reason that the theme is homemade is because if you look closely @ 0:07 to 0:08 you'll see the words "DJ Troopers", which if I'm not mistaken that DJ Troopers is an IIDX theme.
