Normally I use SM5 which has no issues with this (mostly because I can disable Decents / Way Offs) but Beware's Extreme runs on SM 3.9 and it seems to have weird issues with randomly triggering Goods / Boos.
They mostly happen on step patterns where it's alternating between the same two arrows, and the faster the pattern/song is, the more likely it is that a Good/Boo will happen. I tried playing Max 300 Heavy and got nearly 20 goods/boos from this, despite nearly PFCing the song otherwise.
I tried messing with Debounce, and that didn't seem to do much. By cranking it way up, I was able to kinda sorta reduce the number of random goods/boos, but they still happen from time to time, especially on faster songs. PadStickSeconds also didn't seem to make a difference, no matter what I set it to.
Furthermore, I can't see any way to disable the Good / Boo windows, especially since I'm using Stepmania Extreme Online, a fork of Beware's Extreme that specifically disables the ability to alter the timing windows.
Taking a look at my stream of it, it seems that arrows are somehow triggering super early, which causes the Boo because of Extreme's massive Boo windows:

Solution ended up being to increase the debounce time on the pad itself, not stepmania. My pad uses an Arduino so I just went into the code that I used and changed the debounce from 5ms -> 50ms.

Ooh, you know, thanks for troubleshooting this and posting the fix yourself, regardless of the lack of help. I used to get missteps in 3.9+ on my pads, and no one would take the time of day to troubleshoot with me because it wasn't SM5. I'll have to try this out and see if it works for me too, since I struggled with this for 4 years on multiple machines. Would love to have my 3.9+ setup running again.

Yeah, adjusting the debounce in Stepmania (by editing the config file in the SM folder) didn't seem to help, but now that I've increased the debounce on the pad itself, that seems to have fixed it.
So I guess the solution to this is trying to adjust debounce on the pad itself, if you can.
