How about Long Train Runnin' by X-Treme on DDR SN1 Arcade? I don't know if it was just me, but it seemed offsync to me.
And, I've been told iFutureList (also in SN1 Arcade) is offsync. Could someone here please confirm this?

I will argue until the end of time that Unreal from SN2 is ridiculously off sync.
That's Unreal! (It's fine)

Wasn't This Is The World We Live In from Dancing Stage Max unplayable?
I didn't recall this one being too bad, but it's been about 7 years since I last played it so I might be looking at it through NostalgiaVision.A lot of the licences from the European releases, particular the 70s-80s licences were badly synced, particularly A Town Called Malice from Unleashed 2. Which is a shame because for one of my flat-out favourite songs, they wussed out with both a terribly synced chart AND a 5 because they were too scared to use 24ths at 102bpm.

We can't forget about Kakumei as well. And isn't I Feel... Offsync as well on Extreme?

Well enough about off-sync songs, which songs do you think are dead on solid when it comes to syncing? Which DDR song would you calibrate your StepMania with? Do You Remember Me by Jenny from DDRMAX comes to mind...

I remember both Theory of Eternity and CG Project from HP5 being pretty solid. They were the rare cases of being 12s I could get decent PA on so they must count for something.

PARANOiA from 1st Mix is pretty much dead on, at least to me.
