For some reason specifically the DDR and Pump It Up category banners do not work. I have tried various things to see what is wrong. Deleting the cache, swapping images, changing folder names, but for some reason 3.9 AMX discriminates against the DDR & PIU category banners. Is there any way to get them to work?

Don't use SM AMX. Very buggy.

Nothing else supports 3.9 BGanimations, changing game mode within the song list, and Pump It Up styled arrows.

That's a strange requirement list, but I'm pretty sure SM5 has been able to accomplish all of that since at least beta 1.

I tested the latest version of Stepmania 5. It can't run 3.9 BGanimations. Also there are less themes and note skins available to Stepmania 5.
