Ultramix 2 had you enter a Knami Code variant during the credits.

I got bored of trying to figure out what was going on in Konami's head when they programmed this game, so I just did it the easy way. =/
EDIT: Also, there's absolutely no sign of a 5th image. These four are together on the same texture, along with things like the "Complete!" message. If there was a fifth, there would most likely be some indication of it on here.

I'm not sure why this picture wasn't available until now, but thanks anyway. At least we now know what the entire picture looks like.

Apologies for the huge necrobump, but this one is well-deserved, I promise.
Because, now that I have access to this game again with 1000+ arcade MFCs under my belt, I decided to take a crack at beating this challenge once and for all. I'm not sure if I'm the first person ever to accomplish this legitimately, but regardless, I finally did it.
Also, as a bonus, here's gameplay footage of the FREE MISSION MODE that unlocks as a result.
I'm not sure why this picture wasn't available until now, but thanks anyway. At least we now know what the entire picture looks like.

I don't know what's more nuts, the fact that they hid more unlockable content behind such an ludicrously difficult goal, or that said content is such a plain, poorly thought through reward that should have just been the game's standard mission mode, much like the one in Extreme CS.

Eh, I'd hardly call the secret mode new content if it's only purpose is to make it more convenient to access all the missions. No extra missions or anything else significant. It's even more useless now if you've already cleared all of them in the first place. I agree that it should've been unlocked after clearing the final mission (the roppongi EVOLVED train).

After clearing the last DMM mission, there is a notification that confirms FMM is unlocked (nothing to do with Request Mode). After unlocking the last square in Request Mode, the "COMPLETE!" graphic that was speculated/mined earlier in the thread appears, but otherwise . . . nothing (disappointing, I know).

On the same topic of "stupid hard stuff in X2 PS2 that no one probably bothered to clear", I don't think anyone's ever cleared or even seriously attempted to get to the last Encore Extra Stage.
This is because according to footage ([1], [2], [3]), the game uses Supernova 2's Extra Stage system, where the five bosses get placed in a queue and the first not-yet-unlocked song becomes the special Final Stage, the second becomes the Extra Stage, and the third becomes the Encore Extra Stage.
This means that in order to even see the last Encore Extra Stage (roppongi Evolved)...you must first AA Dead End Groove Radar Special under Extra Stage conditions. The arcades wouldn't even get a mandatory 18 as an Extra Stage until six years later.

That is correct. I own that game and remember how stupid that was.

I thought the order was Dance Dance Revolution -> Pluto the First -> Kimono Princess -> DEGRS -> roppongi Evolved.
In the footage, the first video shows Kimono Princess as an Encore Extra Stage at some point. Then the second and third videos show that unlocking everything gives you Kimono Princess as Final Stage and DEGRS as Extra Stage.

I remembered wrong and got confused on what the footage was supposed to be showing off. You're right, the final 5 songs on the song wheel are ordered by when they are unlocked and played as final / extra / encore extra.
